What do I need to bring to my appointment?

  • Referral letter from your GP

  • Medicare card, concession card or DVA card

  • Private health insurance details

  • Medical records of previous surgeries

  • Recent discharge summaries if you have been admitted to hospital

  • Relevant test results including relevant medical reports, blood and urine tests and any imaging (Ultrasound, MRI, CT, X Ray)

  • A list of current medications

  • A list of any questions or concerns you wish to raise in the appointment

  • You are also always welcome to have a friend or family member accompany you for your appointment.

Please let reception know as soon as possible if you are unable to attend your scheduled appointment. Providing early notice helps us manage our appointment schedule effectively and allows booking of another patient to facilitate their care. It also allows us to offer you another appointment without unnecessary delays to your care.

Fees for Consultations

Full payment is required at the time of initial or review consultation. Payment is expected on the day of consultation using EFTPOS (Visa, Mastercard) or cash.

Fees for Surgery

Should surgery be advised, a full discussion of informed financial consent will be undertaken.

Should the procedure be in the private hospital system, there may be additional costs to consider in addition to what is covered by health insurance.

How soon can I see Dr Cole-Clark after a referral is made?

Following receiving your referral the rooms will contact you to arrange an appointment time. If there is concern regarding an urgent condition then Dr Cole-Clark is happy to be contacted by your GP and will endeavour to see you in an urgent manner. 

Do I need blood or imaging tests prior to my appointment?

Depending on the indication for referral you may require additional blood or imaging tests. An example of this is for those who have an elevated PSA to have a repeat PSA level conducted 1-3 months following the initial raised level. Should further investigations be required then appropriate forms will be arranged at the time of confirming your appointment.

Do I need private health insurance?

Dr Cole-Clark operates in both public and private hospitals within Newcastle and the Hunter Valley. He offers surgery within the public hospital for those without private health insurance.

What should I expect during my appointment?

Dr Cole-Clark will review your previous history, symptoms and investigations. You may require further examination and investigations prior to determining the need for any procedures.

Depending on your condition you may be asked to attend with a full bladder and see the practice nurse to determine your urinary flow and residual urine volume. You may also be asked to complete a questionnaire of your urinary symptoms.

If you require an operation then discussion regarding your condition, alternatives, risks/benefits and outcomes of surgery will be undertaken. Questions will be reviewed to ensure you have a full understanding of the procedure. Should a cost be associated with the procedure then this will also be advised.

Can I bring family or friends with me to my appointment?

You are welcome to bring a family member or support person to your appointment. Having someone with you can assist with support, taking notes or asking questions. If you plan to bring more than one person, or if there are special requirements, please let the reception team know in advance so we can ensure your visit runs smoothly. 

What should I expect on the day of my surgery?

You will receive an admission time and fasting instructions the day prior to the surgery.

Any additional questions will be reviewed and pending pathology discussed pre-operatively.

If your procedure is a day case then we will ensure that you have appropriate medications and have passed urine prior to discharge. If your operation requires sedation or a general anaesthetic then you will require someone to assist with transporting you home as well as support at home for at least 24 hours following the procedure.
